Cost: $10.00.

Additional filing:

Fill out a Notice of Claim of Paternity and register it with the Vital Statistics Unit of the Department of Health and Welfare, and a notice of your claim of paternity to the child born out-of-wedlock, and your willingness and intent to support the child to the best of your ability.
