Please fill out the following optional information before submitting your rating:




  • Well-Being Outcome 1: Families have enhanced capacity to provide for their children's needs.

    Performance indicators:

    1. Needs and services of child, parents, foster parents

    2. Child and family involvement in case planning

    3. Worker visits with child

    4. Worker visits with parents
  • Well-Being Outcome 2: Children receive appropriate services to meet their educational needs.

    Performance indicators:

    1. Educational needs of the child
  • Well-Being Outcome 3: Children receive adequate services to meet their physical and mental health needs.

    Performance indicators:

    1. Physical health of the child

    2. Mental health of the child
"Substantial compliance" : These outcomes will be rated on the basis of the onsite review. In 95% of the cases reviewed (90% on the first review), these outcomes must be rated "substantially achieved."
